Cancellation Policy:

Aside from medical or emergent issues, I request a 24 hour notice for cancellation of session. If you cancel an appointment with less than 24 hours notice, or fail to show for an appointment, you will be charged $50/ cancellation fee. Please note, Insurance companies will not reimburse for the cost of cancelled/ missed session.

Insurance:

I accept the following insurances: Aetna, Horizon HMH Inner Circle, Horizon PP0, Horizon BCBS managed care, Horizon Medicare Blue, BCBS, Optum, United Healthcare, Oxford, UMR, Medicare, AARP Medicare supplement plans, Out of Network.

Should you decide to use your insurance in any capacity, it is best to contact your insurance companies directly. The following questions might be helpful when navigating the process:

• Do I have mental health benefits? If so, what do they include?

• Do I have out-of-network benefits? If so, might I be able to submit receipts for personal reimbursement?

• Do I have a deductible and has it been met?

• Is there a set number of allowed sessions per calendar year?

• What is the customary rate that your percentage is based on for the 90834 and 90837 billing codes?
